Benjamin Williams, DO
During his time as a family practice physician in Page Arizona, Dr. Williams has extensive knowledge of pediatrics (youth), adult medicine, and geriatrics (elderly). Dr. Williams serves as our chief medical officer (CMO), and oversees all of our providers here at Creek Valley Health Clinic. As a native to our community, Dr. Williams currently resides with his wife and children in Centennial Park. Dr. Williams is a full-time provider, and is accepting new patients.

Nathan Barlow, DMS, PA-C
Nathan Barlow is a certified physician assistant, with experience as a military medic and EMT. Nathan previously served our local community as a volunteer EMT prior to graduating from his program at the University of Washington. Nathan has recently moved back into the community, and is thrilled to serve you and your family. Nathan is one of Creek Valley's full-time providers, working Monday through Thursday, and is accepting new patients.

Rebekah Williams, PA-C
Rebekah Williams is Creek Valley Health Clinic's new Certified Physician Assistant.
She previously worked in Hyperbaric Medicine for 14 years, and took the next step in her career by graduating from The University of Utah's Physician Assistant Program in 2023. Being native to the Short Creek area, she is so grateful and excited to be home, serving the community. -
Joanne Yarrish, CNM, FNP
Joanne Yarrish is both a certified nurse midwife and a family nurse practitioner. Joanne oversees much of our OB/GYN care at Creek Valley Health Clinic, providing prenatal, postnatal, women's health, and family planning services. She also works at the Centennial Park Birthing Clinic, which will be an option for expectant mothers to give birth. With over 25 years of experience in the short creek community, Joanne is excited to continue caring for our community. Joanne typically works 2-3 days each week, and is accepting new patients.

Matt Lasslo, LPC
Matt Lasslo is a licensed professional counselor in Arizona. He has a diverse background in mental health, beginning as a crisis counselor and later working in outpatient behavioral health. He has also served as a clinical director, developing programs and guiding a team in providing integrated care. Matt is passionate about treating both mental and physical health to achieve positive outcomes. He is committed to compassionate and empathetic care. He looks forward to joining the Creek Valley team and serving the Colorado City community.

Peggy Taylor, Psy.D.
Peggy Taylor is a Clinical Psychologist with Creek Valley. She has over 16 years of experience working in the Colorado City area and spent 20 years prior to that in private practice in Temecula, California. Peggy has worked in various clinical settings and specializes in trauma recovery. She is committed to helping clients explore their strengths, gain insight, and achieve their fullest potential.

Born in Guatemala, Ari is an integrated licensed behavioral health professional who has a passion for social work, and can assist you in developing lifestyles that improve overall health and wellness.

Sandy Taylor, CSW
Sandra Taylor is a Certified Social Worker (CSW) for Creek Valley. She was born in California and has lived in Utah for over 22 years. She graduated from the University of Utah with a Masters of Social Work (MSW). She is a Licensed Practical Nurse (LPN) and served as our clinic Care Coordinator. Sandra’s approach to therapy encompasses a variety of evidence-based therapeutic strategies which support the client in expressing the feelings that led them to professional counseling. She encourages clients to recognize and use their strengths to overcome issues, past unresolved trauma and move toward making positive changes in their lives.

Eric Derbidge
Eric obtained his Bachelor's in Psychology and has completed his Master's in Clinical and Mental health Counseling with an emphasis in Marriage and Family Therapy. Eric has spent the last two years building relationships in the community as a practitioner and counselor, helping those he works with recognize their internal strengths and appreciation for life. He moved from Salt Lake City to Fredonia in 2000 and has spent the last 20+ years coming to understand and love the culture and communities in the area.

Michael DeLeon
Michael DeLeon is currently working towards her Masters degree in Clinical Mental Health Counseling with an emphasis on trauma and plans to further her education toward LPC/licensure. She has been a provider in Colorado City for the past two years while advocating for and empowering clients to reach their goals and live the life they envision. She is passionate about behavioral health and impacting people's lives through highlighting their values and strengths while honoring cultural awareness and sensitivity.
