Creek Valley Health Clinic offers discounts designed to make health care services available to families who are eligible based only on family size and household income. Our HealthShare Program is available to all patients, regardless of income or insurance status.
In order to apply for this program, an application must be completed and given to our front-desk representatives. You may view, or print the enrollment application from the link below. Total household income, names of individuals in your household, and dates of birth of individuals in your household are required within the application.
We also have highly-qualified professionals that can review your situation and provide you assistance and guidance with insurance questions, such as eligibility for Medicaid, Medicare, or third-party insurance plans. Click the link below for more information.
